Thank you for choosing One World Healthcare. We are looking forward to meeting you. In order to get to know you better and to learn about your medical history, we need you to provide us with some information. The best way to do this is through our secure, online patient portal. You can click here to create a new account and provide us with all of the information we need. By registering online, you avoid having to complete paperwork.

If you prefer to complete paperwork, however, click here to download a PDF version of our new patient registration forms. If you are coming in to our office, you’ll need to bring these completed forms with you to your appointment.


Visits are scheduled by appointment. We ask that you notify us 24 hours in advance when you are unable to make a scheduled appointment so that we may give that time to someone else in need.

If you have an urgent issue, please call our office. We are always happy to work in patients who need to be seen urgently even when appointment slots are not available.


When calling to get a prescription refilled, we ask that you provide our office with the following information:

  • Pharmacy Name and Phone Number
  • Name of Medication and Dosage
  • Your Name and Phone Number

We typically call prescription refills in within 1 business day. We do not refill prescriptions after hours or on the weekends unless it is an emergency.


All co-payments, deductibles and non-covered items must be paid for at the time of service. We accept cash, checks and credit/debit cards. As a courtesy to our patients, we file insurance claims with your carrier.
